Lister Health Metrics is a remote care application provided by Lister Healthcare Corporation that supports Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Principal Care Management (PCM). It lets enrolled patients record and transmit health readings — such as blood pressure, blood glucose, weight, heart rate, and oxygen saturation — to their care team, and supports ongoing care coordination for patients managing chronic conditions, between in-person visits.

Lister Health Metrics is **not** for medical emergencies. It does not provide real-time, 24/7 monitoring, and readings may not be reviewed immediately. **If you are experiencing a medical emergency, call 911 (or your local emergency number) or go to the nearest emergency room right away.

How do I get access to the app?
Access is provided to patients enrolled in a monitoring or care management program through their clinic. If you have not been enrolled, contact your clinic or email [email protected].
How do I connect my monitoring device?
Follow the in-app pairing steps for your device. If a device won’t connect, email [email protected] with the device make and model.
My reading didn’t appear in the app — what should I do?
Confirm your device transmitted successfully and that you have an internet connection. If the reading still doesn’t appear, contact support. Do not rely on the app to alert your care team in an urgent situation — call your clinic or 911 instead.
I forgot my password / can’t log in.
Use the “Forgot password” option on the login screen, or contact [email protected].
